Falls prevention and home safety

Strategies to prevent falls in and around the home before they happen. And after a fall, working out why it happened and what will reduce the risk, so you can stay safe and independent at home and in the community.

Falls risk assessment using validated screening and performance-based measures, structured hazard assessment, and a written action plan.

Home modifications

Changes to the home, small or large, that improve safety and accessibility. I assess the environment and prescribe modifications suited to how you live and what you want, from a single grab rail to a full bathroom.

Prescription and specification with measurements, plans and photographs; builder and supplier liaison; post-installation review.

Equipment and aids

Practical equipment to support your daily routines, safety and wellbeing, at home and in the community. Trialled where you live, set up properly, and reviewed once you have been using it.

Assessment, trial and prescription, including AT-HM prescriptions under Support at Home. Set-up, training and follow-up review included.

Assessments and reports

A clear, honest account of how someone is really managing day to day, for the family, the GP or the provider. Written in plain language, with practical recommendations.

Assessment and reporting for registered providers, Support at Home support plan reviews, and the functional justification an AT-HM prescription needs.

Reablement and restorative care

A goal-based program to rebuild something you have stopped doing: the garden, the walk to the shops. We agree the goal, work at it over a set period, and measure progress at the start and the end.

Restorative care and reablement, including Restorative Care Pathway episodes. Outcomes measured at start and finish.

Coming home from hospital

A discharge date often arrives before the home is ready. I can review access and equipment before you come home, and liaise with the discharge team so the first week works.

Moving into care

Staying at home is not always possible. I can help work out what a new room needs, what to take, and how to maintain as much independence as the move allows.

Transition support, seating and equipment review in residential settings, and visits to residents in aged care and retirement villages.

Support for family and carers

Usually a daughter or a son, sometimes managing from a distance. I work alongside you with practical education, safe manual handling, and a clear plan for supporting someone at home or in the community.

How it works

Four steps.

A phone call

A short conversation about what has been happening, and whether occupational therapy is the right support. If it isn’t, I’ll point you to who can help.

A visit

We start with a 60 to 90 minute visit, where you live. We walk through the home, I observe how you manage a few everyday tasks, and we decide together what is needed. Family are welcome, and so is the dog.

A plan you can read

A short written plan in priority order, setting out what is recommended, what it involves and who does what. Nothing proceeds without your agreement.

Seeing it through

I work with local suppliers and builders, your care manager and your family to get it done. Then I follow up to check it’s working and see how you’re getting on. Sometimes that’s a visit, sometimes a phone call.

Paying for it

Two ways.

Fee schedule on request.

  • Through My Aged Care. If you have a Support at Home package, or an older home care package, occupational therapy is funded through it and your provider claims it. Equipment and home modifications are funded separately through the AT-HM scheme.
  • Privately. No referral, waitlist or eligibility test. Often the fastest option when something needs attention now.
