Terms of service.

Who you’re dealing with

Basecamp OT is a sole practice run by Shelley L’Green, an occupational therapist registered with the Australian Health Practitioner Regulation Agency (AHPRA), registration OCC0001726181. ABN 95 566 003 531. Professional indemnity and public liability insurance are held and current.

I work with older people across the Northern Beaches and lower North Shore, in private homes, retirement villages and residential aged care.

What I do and don’t do

Occupational therapy assessment, equipment and home modification prescription, falls prevention, reablement, reporting, and support through a move into care.

I don’t provide personal care, nursing, cleaning or transport, and I’m not an emergency service. If something is urgent, ring 000 or contact your GP.

Recommendations are advice. You decide what to act on, and you’re free to disagree with me.

Fees and payment

  • A current fee schedule is provided before any work starts. You’ll know what something costs before it happens.
  • Time is charged for direct work (visits and travel within the service area) and for indirect work such as reports, prescriptions, and liaison with suppliers, builders and providers.
  • Through My Aged Care: invoiced to your registered provider, who claims it against your package.
  • Privately: invoiced to you, payable within 14 days.
  • Third-party costs such as equipment, builders and installers are quoted and paid separately. I don’t take commission from any supplier.

Cancellations

Appointments are held for you, and a late cancellation usually can’t be filled.

  • More than two business days’ notice: no charge, and we’ll find another time.
  • Less than two business days’ notice, or nobody home: the full fee for the booked appointment.

Things happen. If someone is unwell, has gone to hospital, or there’s a genuine emergency, ring me and the fee will usually be waived. This clause exists for repeated late cancellations, not for bad luck.

If I need to cancel, you’ll get as much notice as I can manage and the first available replacement appointment.

Consent

Before we start I’ll explain what the assessment involves and ask for your consent to go ahead, to take photographs and measurements where they’re needed, and to share the report with the people who need it.

You can decline any part of it, and you can withdraw consent at any time. Where someone has a guardian or an appointed decision-maker, consent is obtained from them, and I’ll still explain what’s happening to the person themselves.

Your information

Health information is handled under the Privacy Act 1988 (Cth) and the Health Records and Information Privacy Act 2002 (NSW).

  • I collect only what’s needed to do the work properly.
  • Information is shared with your provider, GP, suppliers or family only with your consent, or where the law requires it.
  • Records are stored securely and kept for the period required by law.
  • You can ask to see your records, and ask for corrections if something is wrong.

Safety and access

Visits happen in your home or residence, so I’ll ask for safe access: a clear path to the door, dogs settled if that’s easier for everyone, and smoking paused during the visit. If a visit isn’t safe to carry out, I’ll say so and we’ll reschedule.

If something isn’t right

Tell me first. Most things are fixable, and I’d rather hear it directly.

If that doesn’t resolve it, you can contact the Aged Care Quality and Safety Commission on 1800 951 822, or AHPRA on 1300 419 495. Raising a concern won’t affect the care you receive.

These terms may be updated from time to time. The version on this page at the date of your appointment is the one that applies.
